If you are a NEW PATIENT,  you can find a new patient appointment time that works for you by clicking “Book Online”. If you initially scheduled your appointment through the zocdoc website, or request a time through the “Book Online” link on this page, our new patients specialists will be reaching out to you by phone, text, or email to obtain more information. The information entered through the Zocdoc website alone is usually not sufficient.

 

PLEASE NOTE, because of the rapidly rising costs due to inflation from supply shortages, staffing shortages, and extra COVID precautions that are not being met by insurance reimbursement, we require a one time $10 registration fee to book your new patient appointment. This fee is non-refundable, but you will not be charged again if the visit needs to be rescheduled. This fee can not be billed to insurance.

 

When you are scheduled and registered as a patient, you will receive an email or text with a link to set up the patient portal. Please set up the portal account for the patient so you can fill out basic demographic and insurance information as well as clinical history forms electronically. Please note, we will need all of that information at least 72 hours before your appointment.  If you are transferring from another allergist, please make sure we have any previous food allergy labs, skin test results, and allergy shot extract information at least 72 hours before the appointment. If you are coming for eosinophilic esophagitis or immune deficiency, please make sure we have the esophageal biopsy results or preliminary immune study results a minimum of 72 hours before the appointment.
